Eligibility: If you are an AMTA member, less than age 60, actively-at-work at least 20 hours a week, and are a U.S. resident*, you are eligible to apply for a monthly benefit amount up to a maximum of $4,000 or 70% of your average monthly income.
Spouse/Domestic Partner Eligibility: Your spouse/domestic partner is eligible as long as he/she is under age 60, actively-at-work at least 20 hours per week, is a U.S. resident*. Your domestic partner is eligible as long as they are registered as a Domestic Partner, or execute a Domestic Partner Affidavit acceptable to the insurer. If both spouses/domestic partners are members they cannot be covered as a member and dependent.

Benefits Paid for Working While Disabled
Covered Residual Disability can help to protect a portion of your income while you continue your recover and transition back to work.
This coverage provides benefits if you return to work after a period of total disability but are still limited by the same injury, sickness, or organ donation. You may qualify if, after satisfying your waiting period and receiving total disability benefits, you return to work on a prt-time or limited-duty basis and experience a loss of earnings of 20% or more as a result of the same condition. Eligibility applies as long as you have not received benefits for the full maximum benefit period for that total disability and you are under age 60.
Waiting Period is the initial, continuous period of Covered Total Disability which must be completed before such Covered Total Disability benefits come initially payable. Choose among four waiting periods: 45, 60, 90, an d 180 days. The longer the waiting period, the less your premium will be.

This feature helps you return to the workforce. If necessary and appropriate, the coverage provides vocational testing and training, workplace modification, prosthesis and job placement. The premium contributions shown reflect the current rate and benefit structure. Premiums increase on the premium date coinciding with or next following the date that a member enters a new age bracket. Premium contributions may be changed by New York Like Insurance Company on any premium due date and any date on which benefits are changed. However: your rates may change only if they are changed for all other in the same class of insureds under this group policy. Fore example, a class of insureds is a group of people all with the same issue age, waiting period, and benefit option amounts are not guaranteed and are subject to change by agreement between New York Life Insurance Company and the AMTA.

Rate Calculation Example:
A 35-year-old would like a $2,000 monthly disability benefit with a 90-day waiting period:
Monthly cost per $100 for a 90-day waiting period for a 35-year-old = $.50
$2,000 monthly benefit divided by $100 (cost per monthly benefit) = $20
$.50 x $20 = $10 monthly cost

FAQs
How long will the AMTA Disability Income Insurance Plan pay benefits?
While other programs may only protect you for a few years, the AMTA’s Disability Income Insurance coverage can continue through age 64 for any covered disabling illness or injury.
What kinds of disability will the benefit pay for?
The AMTA Disability Income Insurance benefit pays valuable benefits if a covered injury or illness leaves you totally disabled and unable to perform the material and substantial duties of your occupation.
You can also collect benefits for residual disabilities that cause a loss of earnings of at least 20% while you’re performing at least one of the material and substantial duties of your occupation on either a part-time or full-time basis. You must also be under the regular care of a physician. Material and substantial duties are those that are normally required in your occupation and cannot be reasonably omitted or modified.
In addition, a disability that causes a loss of earnings of 80% or more will be considered a total disability. Please refer to the Terms tab for more information on exclusions and limitations.
Will I have to pay taxes on my benefits?
Under current tax law, your AMTA group Disability Income Insurance benfits may be tax-free as long as you pay your own premiums with after-tax dollars. That means you will receive the benfit amount you applied for in the event of a covered disability. If you own your own business, you may instead choose to claim your premium (or premiums for your employees) as a tax deduction. Check with your accountant for details and guidance on current tax law and the options available to you.
Will I have to pay premiums while I’m disabled?
After you have been totally disabled for six consecutive months and you begin to receive benefits for total disability, all future premium contributions under the policy will be waived for as long as you receive benefits for that disability.
The Waiver Of Contribution Benefit will end on the contribution date following the date the Covered Disability benefit ends.

Terms
Effective Date
Your coverage will begin on the first of the month following the date your application is approved by New York Life Insurance Company, provided you are Actively-at-Work full-time, and you are actively performing the normal activities of a person in good health of like age, (For NC Residents: A person of good health) on that date.
If you are not performing your normal activities as required, coverage will not become effective until the day you are performing such normal activities provided such date is within three months of the date insurance would have been effective and you are still eligible for insurance. Payment of a premium contribution for insurance does not mean there is any coverage in force before the effective date as specified by New York Life Insurance Company.
Benefits will begin on the first day of a covered total disability after completion of the applicable Waiting Period. Benefits will continue until the earlier of (a) the day the insured ceases to be totally disabled or (b) the end of the maximum benefit period.
Total Disability means disability which: during the waiting period and first 24 months when Total Disability benefits are payable, wholly and continuously prevents You or Your Spouse from performing the material and substantial Duties of Your or Your Spouse’s occupations; and after that, wholly and continuously prevents You or Your Spouse from engaging in Any Occupation.
Waiting Period means the number of consecutive days at the beginning of any one period of Total Disability which must elapse before benefits are payable.
Actively-at-Work means You or Your Spouse are performing all the Essential Duties of Your or Your Spouse’s Occupation for wage or profit on a full-time basis (at least 20 hours per week).
Average Monthly Income means a person’s average monthly wages, salaries, bonuses, commissions, fees and any other amounts received by such person for personal services, including the cost of his or her fringe benefits and share of total surplus. it does not include income from overtime, other extra compensation, interest, dividends, rent, royalties, annuities, other insurance and other unearned income.
Exclusions
The Policy does not cover any Disability or loss caused by:
- intentionally self-inflicted Injury, suicide or attempted suicide, while sane or insane;
- pregnancy or childbirth, except Complications of Pregnancy;
- war or act of war, whether declared or not;
- A disability that: occurs during; is due to; or is related to; the covered person travel in, travel on, fall from or descent from any aircraft while such aircraft is in flight, unless the covered person is traveling: solely as a fare paying passenger on a licensed, commercial, regularly scheduled, non-military aircraft; in a civilaircraft having a current and valid “Standard Federal Aviation Agency Airworthiness Certificate” and piloted by a person with a current and valid pilots certificate with proper ratings for the type of flight and aircraft involved; or in a transport typeaircraft operated by the Air Mobility Command or its successor organization of the United States or similar air service of any;
- Your commission or attempted commission of a felony;
- A disability that is due to or related to service in the military, naval, or air force of any country, alliance or international organization or in a civilian unit which serves such force.
The Maximum Benefit Period for all Covered Disabilities of an INSURED MEMEBER which are due to or related to Mental Disorders and/or Chemical Dependency while such person is insured under the Policy, whether the insurance has been continuous or interrupted, cannot exceed the lesser of: (a) 24 months; or (b) the Maximum Benefit Period for a Covered Total Disability.
Pre-Existing Condition Limitation:
Conditions Prior to Effective Date: We will not pay a Benefit for any loss or period of Total Disability which: begins during the first 24 months of your insurance; and is a result of a Pre-Existing Condition unless such Total Disability begins after You have been free of Medical Care for the condition for a 12 month period ending any time on or after Your effective date..
Pre-Existing Condition means any covered Disability, diagnosed or undiagnosed, for which medical care is received by you: within the 12-month period prior to your effective date of insurance, or prior to any increase in coverage.
Impairment Restriction is a disability that is due to or related to a condition which has an Impairment Restriction. However, at any time and at this or her own expense, the INSURED MEMEBER can give medical evidence of insurability for a condition which has an Impairment Restriction. After review of such evidence, New York Life will determine: (a) if and when such Impairment Restriction should be removed or liberalized; or (b) if it should be continued.
When coverage ends your coverage and your Spouse’s coverage will end on the earliest of:
- You are no longer an AMTA Member
- You are no longer at full-time work
- You stop paying premiums when due
- When you receive covered total disability benefits for the maximum benefit period
- When you being active duty in the armed forces
- The date the group policy ends
- When spouse ceases the be lawfully marries or domestic parrtner
However, if Your Spouse’s coverage would terminate because of Your death, coverage will continue until the premium due date on or next following Your death unless continued in accordance with the Surviving Spouse Continuation provision.
Successive Period of Covered Total Disability will be considered one period of Covered Total Disability if such disabilities are due to the same or related causes, and which are separated by less than 6 months of return to continuous full-time work during which you are not totally disabled or difference or unrelated causes are not separated by return to full-time work for at least one day.
Certificate of Insurance: This information is only a brief description of the principal provisions and features of the policy. The complete terms and conditions are set forth in the group policy issued by New York Life to Trustee under the Trust Agreement with the American Society of Mechanical Engineers. When you become insured, you will be sent a Certificate of Insurance summarizing your benefits.

How New York Life Obtains Information and Underwrites Your Request for the AMTA Disability Income Insurance Plan
Information regarding insurability will be treated as confidential. In considering your request for insurance, we will rely on the medical information you provide, and on the information you authorize us to obtain from your physician, other medical practitioners and facilities, other insurance companies to which you have applied for insurance and MIB, LLC (formerly known as Medical Information Bureau). MIB and other insurance companies may also furnish New York Life, its subsidiaries or the plan administrator with non-medical information (such as driving records, past convictions, hazardous sport or aviation activity, use of alcohol or drugs, and other application for insurance). The information provided may include information that may predate the time frame stated on the medical questions section, if any, on this application. This information may be used during the underwriting and claims processes, where permitted by law.
New York Life may release this information to the Plan Administrator, other insurance companies to which you may apply for life and health insurance, or to which a claim for benefits may be submitted and to others whom you authorize in writing, however, this will not be done in connection with test results concerning Acquired Immune Deficiency Syndrome (AIDS) or Human Immunodeficiency Virus (HIV). We may also make a brief report of your protected health information to MIB, but we will not disclose our underwriting decision.
New York Life will not disclose such information to anyone except those you authorize or where required or permitted by law. Information in our files may be seen by New York Life and Plan Administrator employees, but only on a “need to know” basis in considering your request. Upon receipt of all requested information, we will make a determination as to whether your request for insurance can be approved.
If we cannot provide the coverage you requested, we will tell you why. If you feel our information is inaccurate, you will be given a chance to correct or complete the information in our files. Upon written request to New York Life or MIB, you will be provided with non-medical information. Generally, medical information will be given either directly to the proposed insured or to a medical professional designated by the proposed insured. Your request is handled in accordance with the Federal Fair Credit Reporting Act procedures. If you question the accuracy of the information provided by MIB, you may contact MIB and seek a correction.
